Evidence packages combine forms with photos and signatures on the visit.

Photos and signatures turn a form into auditable proof of work. Field users capture them on the mobile app during a visit; supervisors review them on the web alongside GPS, timestamps, and other answers.

Image fields

Add an Image field (sometimes labeled Camera in older forms) when you need photographic evidence—equipment condition, shelf display, completed repair, serial plate, etc.

In field settings, Allow gallery upload controls the source:

  • Off (default for strict evidence) — only the device camera. Users must take a fresh photo on site.
  • On — users may pick an existing image from the gallery. Useful for reference uploads managed by the office; use carefully when proof-of-presence matters.

Images sync with the visit when connectivity returns. Multiple images may be supported depending on form configuration; each attachment stays tied to its field code in submissions.

Signature fields

Add a Signature field when a customer, supervisor, or technician must acknowledge completion—work orders, delivery receipts, safety checklists, waivers.

On mobile the user draws on a signature pad with finger or stylus. The stroke is stored as image data linked to the visit. Signatures are not editable after submit; plan a clear Clear path on mobile before finishing.

Place signature fields after the work description so signers see what they are approving. Pair with a printed name or ID field when audits require identity.

Evidence in visit review

Submitted photos and signatures appear in:

  • Visit detail on the web (timeline and form answers)
  • Form responses explorer for aggregated review
  • Exports and operational reports

They establish what was documented; visit GPS and timestamps establish where and when the visit occurred—configured at the visit-type level, not as form field types.

PDF preview

Form Builder includes PDF preview for checking how completed answers—including image placeholders and signature blocks—will look in a printable or email-ready approval copy. Open preview from the canvas to verify layout before publishing.

Photos render inline where the template allows; signatures appear in dedicated signature areas. Adjust field order and page breaks if evidence should start on a new page.

Tips

  • Require at least one photo on high-risk tasks; optional photos often get skipped under time pressure.
  • Label image fields specifically (Before photo, After photo) instead of generic Photo 1.
  • Disable gallery upload when policy requires live capture.
  • Test signature fields on the smallest devices your team uses—short pads frustrate signers.
  • Combine signatures with Repeatable sections only when each row needs its own acknowledgment; otherwise one signature at the end is simpler.
