Form Builder settings glossary
Reference for form-level settings in Chekku Form Builder.
Audience: Admin
Form-level settings apply to every submission of a form unless overridden by field rules. This glossary mirrors the Form Builder settings panel—use it when configuring publish, notifications, and visit attachment.
Identity and publishing
| Setting | Description |

| Form name | Display name in builder lists and mobile form picker. |
| Description | Internal note for admins; may appear in mobile help text if enabled. |
| Status (draft / published) | Only published forms sync to mobile. |
| Version | Each publish creates a version; historical submissions retain their version for reporting. |
Visit type attachment
| Setting | Description |
|---|---|
| Linked visit types | Check-in types where this form appears on mobile. |
| Required on visit | User cannot complete visit until form is submitted (when visit type enforces it). |
| Order | Display sequence when multiple forms attach to one visit type. |
Attachment is configured both here and on the visit type side—keep both in sync. See Check-in types.
Notifications
| Setting | Description |
|---|---|
| Email on submit | Send message to fixed addresses or roles when form saves. |
| Include PDF / summary | Attach or link submission snapshot (plan-dependent). |
| WhatsApp on submit | Requires company WhatsApp Business API (Reports and notifications). |
For multi-step routing (tasks, approvals), prefer Workflows with form_submitted over stacking many email rules (Automations on form submit).
Mobile behavior

|---|---|
| Allow save draft | Partial save when connectivity is poor (if enabled for form). |
| Offline queue | Submissions sync when device reconnects. |
| Show in quick actions | Surfaces form on mobile shortcuts for selected visit types. |
Access and roles
| Setting | Description |
|---|---|
| Visible to teams / users | Restrict who sees the form on mobile. |
| Editable after submit | Whether field users can reopen and change answers (use sparingly for audit trails). |
Advanced
| Setting | Description |
|---|---|
| Default language | Base locale for labels; pair with translations if used. |
| Custom CSS / layout | Web preview tweaks; mobile uses Chekku native layout. |
| API exposure | Submissions addressable via REST using field keys. |

Tips
- Publish only after mobile test.
- One notification path per event—Workflow or email or webhook, documented.
- When retiring a form, unlink visit types before deleting to avoid mobile errors.
