Form Builder v2 organizes fields into palette groups. Only the types listed here are available to add today. Use this catalog when planning forms and training admins.
Structure
These fields organize layout; they do not collect answers (except inside nested groups).
Header
Section title or instruction block. Use it to separate phases of a long form (“Customer info”, “Equipment check”).
FieldSet
Groups related fields. Can be configured as a repeatable section so mobile users add multiple rows (line items, equipment readings). Child fields live inside the set.
Basics
Standard text and date inputs.
Text Box
Single-line text—names, codes, short notes.
Number
Numeric input. Can feed Formula fields. Supports decimals according to field settings.
Text Area
Multi-line text for observations, comments, or detailed descriptions.
Date Picker
Date or time selection. Can be configured for date-only or time-only capture.
Paragraph
Static explanatory text shown to the respondent. Not editable on mobile.
Selection
Choose one or many predefined options.
Checkboxes
Multiple selections from a list you define. Options can display horizontally on wide layouts.
Radio Buttons
Exactly one selection from a list. Use when choices are mutually exclusive.
Advanced
Connected data and calculated values.
List
Dropdown bound to a Data Source. Shows rows from your configured catalog (products, assets, customers). Filters can limit rows by form or visit context.
Formula
Read-only calculated field. Reference number fields, list columns, and other formula sources. Updates as the user fills the form.
Media
Evidence capture.
Image
Photo capture from the device camera or gallery (when gallery upload is enabled). Use for site photos, damage documentation, or delivery proof. There is no separate Camera field in the palette—Image covers capture.
Signature
Touch or stylus signature pad. Can be marked read-only when pre-filled from a variable.
Not available yet
The following appear in product plans or as disabled previews but cannot be added to new forms today:
- Dropdown (single select without data source)
- Tags
- Rating
- Range slider
- Multi-column row layouts (two-, three-, or four-column rows)
Do not document or train field teams on these until they appear as active palette items.
What is not a form field
GPS location is captured automatically with the visit check-in, not as a form question. Do not expect a GPS field type in the builder.
Tips
- Prefer List over free text when answers must match a master catalog.
- Put Formula fields after the numbers they reference.
- Use Header + FieldSet for repeatable line items instead of duplicating questions.
- Test Image and Signature on a real phone—emulators miss camera behavior.
